Use EPA Form 1 (general information) and the applicable Form 2 (industry-specific or municipal). Provide facility identification, discharge outfall locations, pollutant characterization data, and flow measurements. Include treatment process descriptions and monitoring data. NPDES permits are issued for 5-year terms and must be renewed 180 days before expiration. Many states have delegated NPDES authority and may have additional requirements.
