  • Tenant Information: Full name of all tenants on the lease and property address
  • Reason: Specify the reason — non-payment of rent, lease violation, or no-cause termination
  • Cure Period: If applicable, state the number of days the tenant has to fix the issue (varies by state: 3-30 days)
  • Amount Owed: For non-payment, specify the exact amount of overdue rent
  • Deadline: State the date by which the tenant must vacate or cure the violation
  • Service: Serve the notice according to state law (personal delivery, posting on door, certified mail)
    • Important: Self-help evictions (changing locks, removing belongings, shutting off utilities) are illegal in all 50 states. You must go through the court process.
