File concurrently with or after an approved I-130 or I-140 petition (if a visa number is immediately available). Include Form I-693 medical examination, two passport-style photos, copy of passport and I-94, evidence of lawful entry, and any required waivers. Filing fee is $1,440 (includes biometrics for ages 14-78). Applicants may file Form I-765 (work permit) and I-131 (travel document) concurrently at no additional fee.
