Instructions
Complete all fields including generator name and EPA ID number, transporter information, designated facility, waste descriptions with EPA waste codes, quantities, and container types. The manifest must accompany the shipment from cradle to grave. Generators must sign and retain copies. If a signed copy is not returned by the designated facility within 35 days, initiate an exception report. Electronic manifests are available through EPA's e-Manifest system.
