Specify whether you wish to donate all organs and tissues or limit donation to specific organs. You may designate the donation for transplantation, therapy, research, or education. Sign the document and have it witnessed by two adults. Register with your state's donor registry. Indicate your donor status on your driver's license. Inform your family of your wishes. The directive can be revoked at any time.
