Petition for Guardianship of an Incapacitated Adult

Identify the proposed guardian and the alleged incapacitated person (AIP). Provide medical evidence of incapacity from a licensed physician. Explain why less restrictive alternatives (power of attorney, supported decision-making) are insufficient. The AIP has the right to an attorney, an independent evaluation, and to be present at the hearing. Courts prefer limited guardianship over full guardianship when possible.
