Instructions

Notify your employer in writing of the conflict between your religious practice and a work requirement. Describe the specific religious belief or practice and the accommodation you need (schedule changes, dress code exceptions, prayer breaks). The employer must provide a reasonable accommodation unless it would cause an undue hardship on the business. Following Groff v. DeJoy (2023), undue hardship means a substantial burden on the conduct of the employer's business. You do not need to belong to an organized religion.
