Instructions
Address the letter to the school principal or director of special education. Describe your concerns about your child's academic performance, behavior, or development. Request a comprehensive evaluation under IDEA. The school has 15 school days to respond with consent to evaluate or a written refusal with reasons. Once consent is given, the evaluation must be completed within 60 days. Keep a copy of your dated request.
