Student Athlete Eligibility and Compliance Form

Complete all sections including academic standing verification, amateur status certification, drug testing consent, and medical clearance. NCAA Division I and II athletes must register with the NCAA Eligibility Center. Verify that the student meets minimum GPA and credit hour requirements. Disclose any prior participation at other institutions for transfer eligibility purposes. The form must be signed by the student, a parent (if under 18), and the athletic compliance officer.
