Instructions

File between the 9th and 10th year after registration (and every 10 years thereafter). A Section 8 declaration of use must also be filed between the 5th and 6th year. Provide a specimen showing current use of the mark for each class of goods or services. Identify any goods or services you are no longer using. Filing fee is $225-$525 per class depending on the form used. Failure to file results in cancellation of the registration.
