Employers with more than ten employees in most industries must maintain records of work-related injuries and illnesses on OSHA Forms 300, 300A, and 301. Employers must report work-related fatalities within 8 hours and work-related inpatient hospitalizations, amputations, or losses of an eye within 24 hours.
The annual summary (Form 300A) must be posted in a visible location in the workplace from February 1 through April 30 of the following year. Employers must retain records for five years.
